If you have already been seen by one of our clinicians and you would like to make another appointment, please contact your clinician directly or go to our secure online scheduling system, www.schedule.care. Use the login credentials provided by your provider to access the secure online scheduling system. If it has been an extended period of time since your last appointment you may need to have your password reset. Contact your provider if you need assistance logging in.

Need to Cancel?
Feel free to cancel an appointment if needed; however please make every effort to do so 24-hours before your scheduled appointment. If you are unable to keep your scheduled appointment and fail to cancel within 24 hours, you will be billed $60.00 for a missed session. Please contact your provider directly or use our online scheduling system to cancel or change your appointment.
515 Therapy & Consulting Payment Options
Payment is due at the time of service. You will pay your clinician directly at the end of each session. Cash and credit are accepted. Please contact your clinician regarding payment questions or contact our Billing Specialist at 515.402.4395.
For outstanding balances, an easy way to pay is by going online to www.schedule.care - this website is also used to schedule appointments. Once you login, click on the button, View or Pay Online Statement. If you are unsure of your login information contact your clinician.
You may also leave your credit card information on your clinician’s confidential voicemail by dialing their direct phone line.
