515 Therapy and Consulting accepts Blue Cross Blue Shield, Health Partners, Midland's Choice/Cigna, Aplos, United Healthcare (including Medica, UMR, Surest, Oscar - limited providers).
If your insurance is not listed, you may be able to have a portion of the services reimbursed by your insurance, depending on your out-of-network benefits (see our Insurance FAQ page). Self-pay is always an option. Payment is due at the time of service. Cash and credit are accepted.
